Position Information FTE: 1.000000 (40hrs/week) Exempt: Yes Day Shift: Hours per schedule Weekend & Holiday Rotation: Per schedule Position Summary This position provides leadership and consultation in all aspects of care management: episodic case management (home care, long term care, outpatient and inpatient rehabilitation, acute inpatient) and ongoing care management of patients across the continuum. This position is responsible for the development, implementation, direction and evaluation of an operational plan for care management that is in alignment with the organizational strategic plan to ensure that processes meet or exceed regulatory and industrial standards and result in improved patient outcomes and appropriate utilization of services. The position oversees a centralized care management infrastructure with aligned resources across MidMichigan Health. Responsibilities ESSENTIAL DUTIES AND RESPONSIBILITIES (25%)* Monitors and oversees all care management responsibilities across MidMichigan Health. (20%)* Oversees the Continuing Care Provider Practice (includes Palliative Care and Geriatrics), collaborating with post-acute care leadership and MidMichigan Physicians Group. (15%)* Development, implementation, direction and evaluation of an operational plan for ?care management that ensures improved patient transitions and outcomes with appropriate utilization of services. (15%)* Ensures highly effective operations for episodic case management (including patient placement, utilization review, discharge planning, and social work) by development, continual process improvement and coordination of processes across all functions. (15%)* Develops and implements programs and activities to optimize patient length of stay. Ensures timely patient throughput, appropriate discharge planning while engaging the patient to reduce readmissions and pursues creative alternatives to inpatient admission. (10%)* Assures high risk patients are identified for care management by developing and standardization of protocols and processes to assure access and appropriate transitions of care. OTHER DUTIES AND RESPONSIBILITIES Develops programs, provides education and processes to encourage patient, family, physician and community engagement in the care management process. Evaluates staffing coverage based upon established case management ratios to serve populations needs, volume and acuity of patients. Creates care management outcome priorities and establishes ongoing monitoring processes to assure success. Ensures provision of quality services by maintaining appropriate resources, staffing levels, competency of staff and ongoing training and education. Provides leadership for collaboration with indirect reports in care and case management roles in the outpatient setting. Develops, monitors, evaluates and redesigns practices and policies/ procedures as needed. Responsible for performance management of direct reports. Provides timely evaluation of staff performance and gives direction for their continued growth and development. Oversees hiring, disciplining and discharging employees within areas of responsibility according to MidMichigan Health policy and procedure. Oversees the development of the department budget and ensures alignment with departmental financial targets. Demonstrates leadership behaviors to support change, including the recognition of readiness, change management, collaboration and managing the environment of change. Assures compliance with accrediting agencies, CMS regulations, and MDPH standards and conditions of participation. Employee may be required to participate in further learning opportunities offered by MidMichigan Health Qualifications * Bachelors Degree in Nursing required * Masters Degree preferred * State of Michigan Registered Nurse Licensure * Attainment of certifications in Case Management and Care Management within three years of eligibility required. EXPERIENCE, TRAINING AND SKILLS Minimum 3 years direct management of employees required. Interpersonal skills needed to communicate successfully with individuals and groups to interact with people at all levels to communicate ideas and concepts in a clear and understandable manner. Minimum 3 years case/care management experience preferred Physical/Mental Requirements and Working Conditions Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death. Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position. Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description. Overall vision and hearing is necessary with or without assisted device(s). Frequently required to sit/stand/walk for long periods of time. May require frequent postural changes such as stooping, kneeling or crouching. Some exposure to blood borne pathogens and other potentially infectious material. Must follow MidMichigan Health bloodborne pathogen and TB testing as required. Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake. Overall dexterity is required including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis. Physical Demand Level: Sedentary. Must be able to occasionally (0-33% of the workday) lift or carry 0-10 lbs.SDL2017
