Summary

GENERAL SUMMARY:

The Nurse Navigator serves as a focal point for women entering the health system with breast problems or breast cancer. The Nurse Navigator coordinates services across and within the breast health program. The Nurse Navigator assists the patient in plotting a course through the often complicated system of care. The Nurse Navigator is an allied health professional licensed as a registered nurse by the State of Michigan. This position serves as the single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive breast care services offered within MidMichigan Health System and within its respective communities. The Nurse Navigator facilitates patient appointments, including lab, diagnostic testing and specialty physicians.

(30%)* Assist patients with navigation of the health system and lingo from point of diagnosis through their courses of treatment. Counsel patients and families on their medical condition, individual plan of care, and use of prescribed treatment/medications. Coordinate patient care with other treatment team members. Possess an in-depth knowledge of breast health, diagnoses, and disease processes treatment options. Coordinate appointments for consultations and physician visits.

(30%)* Identify and develop resources to meet the educational and psychosocial needs of women. Connect patients with supportive services that are available through MidMichigan Health and in the community.

(30%)* Develop relationships with referring physicians and educate them on the role of patient navigation/linkage and the assistance that can be provided by the Nurse Navigator. Maintain two-way communications with physician offices in order to improve processes of coordinating and streamlining care for all referred patients with a focus on breast cancer.

(10%)* Develop standardized documentation for the navigation program. Maintain accurate, complete and timely documentation of all encounters with patients and provide data/reports as needed on the navigation program. Ongoing evaluation of patients health status, utilizing appropriate resources as needed.

OTHER DUTIES AND RESPONSIBILITIES:

Work with other MidMichigan team members to develop QA measures for the program to assist in streamlining the path of care for patients from diagnosis through treatment.

Responsible for the ongoing performance improvement in the following areas as appropriate: clinical care outcomes, customer/service improvement, process flow through the health system, etc.

Have a presence in both the professional and general communities to ensure high visibility of the MidMichigan Health Programs.

Responsible for outreach efforts to establish and maintain positive working relationships with key stakeholders (physicians, office staff, diagnostic staff, nurses, ancillary departments, medical social work staff, and support agencies within respective communities).

REQUIRED EDUCATION, EXPERIENCE, TRAINING AND SKILLS:

Associates Degree required, Bachelors Degree preferred.

Registered Nurse licensure in the State of Michigan required.

Oncology Nursing Society certification as a Certified Breast Care Nurse (CBCN) required within two years of hire or transfer into the position.

BLS required.

Must have problem solving skills, excellent phone skills, ability to work as a team member, computer skills in word processing, spread sheets and graphics.

Must have strong multi-tasking skills.

Knowledge of Microsoft office, RIS, electronic medical record, ability to handle tasks and projects, fax and complex.

PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS:

Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death.

Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.

Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description.

Overall vision and hearing is necessary with or without assisted device(s).

Frequently required to sit/stand/walk for long periods of time. May require frequent postural changes such as stooping, kneeling or crouching.

Some exposure to blood borne pathogens and other potentially infectious material.

Must follow MidMichigan Health bloodborne pathogen and TB testing as required.

Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake.

Overall dexterity is required including handling, reaching, grasping, fingering and feeling.

May require repetition of these movements on a regular to frequent basis.

Physical Demand Level: Light.

Must be able to occasionally (0-33% of the workday) lift or carry 11-20 lbs., frequently (34-66% of the workday) 10 lbs. and or Walk/Stand/Push/Pull of Arm/Leg controls.
