Job Description:
Summary General Summary The MSW plays a pivotal role in carrying
out the mission of the organization providing high quality care to
patients who are experiencing crisis in the Emergency Department.
The MSW must have a high level of expertise and skill to create
highly effective strategies for short term crisis management and
suicide prevention, along with helping patients with longer term
goals to prevent high utilization of the Emergency Department or
inpatient admission by providing tools for the patient to use in
everyday life handling environmental and social situations. MSW
must be proficient in planning, executing and monitoring the
effectiveness of care provided to ensure timely transition through
the continuum of care. The social worker must also have skill in
educating and consulting with the treatment team to ensure accuracy
of medical necessity criteria and to ensure timely and appropriate
intervention using a patient centered approach. This requires
effective skills in training, mentoring, decision making and team
building. The social worker is responsible for assessing patients'
psychosocial, spiritual, educational and counseling needs, and
assisting patients who are experiencing behavioral health crisis to
move through the crisis to a path of recovery.
Essential Duties & Responsibilities
(60%)* Provide social work
intervention in the emergency setting as it relates to possible
inpatient hospitalization, crisis management, decision making,
illness adjustment, ethical and legal concerns, discharge and
transitional care needs, domestic violence and abuse, competency,
financial problems, compliance issues, substance abuse and other
psychosocial barriers to maximizing health status. (20%)* Work with
the health care team, patient, family and/or significant others,
community resources to support the patient in transition through
the crisis situation and assuring appropriate level and place of
care. Advocate, mediate and negotiate with an emphasis on
self-determination for the patient. Play an integral role in
development of a cohesive plan to maintain the patient's health
status, prevent suicide, and improve social supports while in the
least restrictive environment and being fiscally responsible.
(20%)* The MSW collaborates with the members of the
interdisciplinary team on an ongoing basis in the planning,
assessment, implementation and evaluation of the crisis plan, with
the goal of formulating a solid plan for aftercare and follow up
services.
OTHER DUTIES AND RESPONSIBILITIES:
Complete and document patient crisis
assessment, provide safety planning, and follow the patient to
determine if means reduction took place and follow up care is
received. Identify the need for and participate in family meetings
as appropriate in order to ensure understanding, comfort and
decision- making for patient's safety plan. Integrate relevant
theories of family dynamics, crisis intervention, strengths-based,
solution-focused and meaning of illness into every day work with
patients. Participates in care coordination efforts to identify any
ongoing high-risk factors and respond appropriately so that key
information is captured and documented. Participates in team
meetings as appropriate. Performs accurate documentation of
interventions and patient progress according to established
standards and hospital policy. Coordinates care with outside
agencies to ensure appropriate follow up care after crisis. Builds
professional relationships with patients, families and the
Emergency Department and Behavioral Health Unit team to address
conflict resolution with positive outcomes. Communicates teamwork,
caring and compassion. Provides excellent customer service in
accordance with the mission, vision and values of the health
system. Assume responsibility for continuing professional
development and maintains licensure. Provides for the age specific
needs of the population served according to evidence based
standards and organizational policies/procedures for the adult and
older adult populations. Other duties as assigned Membership in at
least one professional organization (i.e. NASW, SSWLHC-MI Chapter,
national SSWLHC, etc.) preferred. REQUIRED EXPERIENCE, TRAINING AND
SKILLS:
Mandatory Recipient Rights training
within 30 days of hire and annually is required. Training and
experience, and/or demonstrated competency in casework principles,
including diagnosis, assessment, crisis intervention, treatment and
transition planning; techniques in motivational interviewing and
short-term, solution-focused therapy; awareness of community
resources, public assistance and entitlement programs, ensuring
transition to appropriate levels of care; and issues relating to
age and stage of development, special needs and cultural patterns
of patient population served. Knowledge of social work practices
and processes related to the care of the behavioral health
patients. Interpersonal skills needed to communicate successfully
with individuals and groups and interact with people at all levels
to communicate ideas and concepts in a clear and understandable
manner. Must adhere to the National Associate of Social Workers'
Code of Ethics. Must follow MidMichigan Health policies and
procedures and The Michigan Mental Health Code. Must meet mandatory
educational competency requirements. MidMichigan Health is a

MidMichigan Health. PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL
WORKING CONDITIONS:
Exposure to stressful situations,
including those involving public contact, as well as, trauma, grief
and death. Able to wear personal protective equipment that includes
latex materials or appropriate substitute if required for your
position. Is able to move freely about facility with or without an
assisted device and must be able to perform the functions of the
job as outlined in the job description. Overall vision and hearing
is necessary with or without assisted device(s). Frequently
required to sit/stand/walk for long periods of time. May require
frequent postural changes such as stooping, kneeling or crouching.
Some exposure to blood borne pathogens and other potentially
infectious material. Must follow MidMichigan Health bloodborne
pathogen and TB testing as required. Ability to handle multiple
tasks, get along with others, work independently, regular and
predictable attendance and ability to stay awake. Overall dexterity
is required including handling, reaching, grasping, fingering and
feeling. May require repetition of these movements on a regular to
frequent basis. Physical Demand Level: Light. Must be able to
occasionally (0-33% of the workday) lift or carry 11-20 lbs.,
frequently (34-66% of the workday) 10 lbs. and or
Walk/Stand/Push/Pull of Arm/Leg controls.
