Job Description:
Summary GENERAL SUMMARY
The Patient Throughput Coordinator serves as the hospital lead for
managing daily patient throughput. Responsible for guiding the team
process and tasks while building collaboration with the
patient/family and multi-disciplinary team. The primary goals of
this position are to improve hospital throughput, increase
inpatient bed utilization and reduce patient or bed turnaround
delays. Uses EMR documentation, rounding and communications with
staff to maintain an understanding of house-wide bed supply and
demand. Partners with Case Management, nursing unit staff,
attending providers and any appropriate ancillary services to plan
for upcoming discharges. Rounds to the inpatient floors and uses
various communication methods to assess and resolve throughput
issues. Leverages clinical decision-making in working with the
multi-disciplinary team to ensure timely discharge planning occurs.
Acts as a leader in coordinating and communicating the hospital
plan course as well as discharge plans/needs of admitted patients.
Facilitates and enhances communication between providers, staff,
and unit leaders, and serves as a value-added resource to
streamline through the health care system. Facilitates operations
that support high quality, safe, and efficient handoffs and patient
throughput excellence.
ESSENTIAL DUTIES AND RESPONSIBILITIES
(50%) * Manages and communicates plans for hospital course based on
collaboration with the attending physician and ensures the
multi-disciplinary team, patient/family, and appropriate post-acute
referral sources are aware of the plans for discharge.
(30%) * Actively engages in discussions with unit nurses, case
managers, attending physicians, ancillary team members,
patients/family members, and post-acute referral sources on a
routine basis.
(10%) * Completes all documentation and data entry requirements in
EPIC.
(10%) * Communicates with outside agencies to ensure accurate
information is received to facilitate discharge plans as
anticipated.
OTHER DUTIES AND RESPONSIBILITIES
Uses critical thinking in all interactions.
Sets priorities so that patient needs are met in a timely way.
Prioritizing discharges over the next 24-48 hours.
Timely EDD updates.
Identify patients for the Transitions of Care clinic. Serves as a
clinical resource for the hospitalist practice manager and
hospitalist leaders.
Interacts positively with personnel and shows respect for
physicians and other members of the team.
Builds professional relationships with patient, families,
facilities and resources.
Other duties as assigned.
CERTIFICATIONS AND LICENSURE
RN - Registered Nurse Required
EXPERIENCE, TRAINING AND SKILLS
Required minimum 2 years of case management experience.
Strong interpersonal skills, oral and written communication and
analytical problem-solving skills.
Strong understanding of hospital systems processes and flow.
Proven experience in conflict management and strong ability to
negotiate differences.
Basic word processing, e-mail, and computer knowledge skills.
Organizational, analytical, writing and interpersonal skills.
Dependable, self directed and pleasant.
Critical thinking, problem solving, and deductive reasoning
skills.

PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS
Exposure to stressful situations, including those involving public
contact, as well as, trauma, grief and death. Able to wear personal
protective equipment that includes latex materials or appropriate
substitute if required for your position. Is able to move freely
about facility with or without an assisted device and must be able
to perform the functions of the job as outlined in the job
description. Overall vision and hearing is necessary with or
without assisted device(s). Frequently required to sit/stand/walk
for long periods of time. May require frequent postural changes
such as stooping, kneeling or crouching. Some exposure to blood
borne pathogens and other potentially infectious material. Must
follow MyMichigan Health blood borne pathogen and TB testing as
required. Ability to handle multiple tasks, get along with others,
work independently, regular and predictable attendance and ability
to stay awake. Overall dexterity is required including handling,
reaching, grasping, fingering and feeling. May require repetition
of these movements on a regular to frequent basis. Physical Demand
Level: Light. Must be able to occasionally (0?33% of the workday)
lift or carry 11?20 lbs., frequently (34?66% of the workday) 10
lbs. and or Walk/Stand/Push/Pull of Arm/Leg controls.
