Summary
The Nurse Navigator serves as a focal point for men and women
entering the health system with urologic cancer. The Nurse
Navigator coordinates services across the continumm of care within
urology services. The Nurse Navigator assists the patient in
plotting a course through the often-complicated system of care. Is
an allied health professional licensed as a registered nurse by the
State of Michigan. This position serves as the single point of
contact for referring physicians, patients, and caregivers to
provide resources and assistance with accessing urology services
offered within MyMichigan Health System and within its respective
communities.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Maintain twoway communication with physician offices in order
to improve processes of coordinating and streamlining care for
patients with a history of urologic cancer, concern for urologic
cancer or prostate cancer. The nurse navigator provides information
to the tumor registry for urologic cancer conferences. The nurse
navigator completes the tumor board notes in the patient record and
follows up with recommendations made at tumor board by ordering any
additional testing, disclosing test results and making referrals as
directed
- Assist patients with navigation of the health system and lingo
from point of diagnosis through their courses of treatment. Counsel
patients and families on their medical condition, individual plan
of care, and use of prescribed treatment/medications. Coordinate
patient care with other treatment team members. Possess an indepth
knowledge of urologic cancer, diagnoses, and disease processes
treatment options. Coordinate appointments for consultations and
physician visits. The navigator orders additional testing in
consultation with the urology providers and with provider guidance,
discloses testing results to the patients and enters additional
referrals as needed.
- Identify and develop resources to meet the educational and
psychosocial needs of patients with urologic cancer. Connect
patients with supportive services that are available through
MyMichigan Health and in the community, including setting patients
up with the Prostate Cancer Survivorship Program provided by
Michigan Medicine.
- Provides follow up surveillance plan to patients and the care
team for cystoscopies, CT scans, MRI and lab testing. Monitors
patients follow through with surveillance/treatment plan.
- Develop standardized documentation for the navigation program.
Maintain accurate, complete and timely documentation of all
encounters with patients and provide data/reports as needed on the
navigation program. Ongoing evaluation of patients' health status,
utilizing appropriate resources as needed. Works collaboratively
with the urology MA leading MUSIC data abstraction and submission,
specifically for renal cell and prostate cancer.
REQUIRED CERTIFICATIONS AND LICENSURE
- BLS- Basic Life Support
- RN- Registered Nurse BSN
- Comments: Enrolled in BSN program
REQUIRED EDUCATION
- Education: Bachelors Degree or enrolled in BSN program
- Comments: 3-5 years experience in urology or oncology related
experience
