Conduct thorough evaluations of speech, language, voice, cognition,
and swallowing abilities - Develop and implement personalized
treatment plans in collaboration with patients and the care team -
Deliver home-based therapy sessions following physician orders and
evidence-based practices - Educate and empower patients and
caregivers with home programs to reinforce therapy goals - Maintain
accurate and timely clinical documentation - Communicate
effectively with physicians, nurses, and therapists to ensure
continuity of care - Identify patient changes early to prevent
complications and unnecessary hospitalizations - Promote patient
safety, independence, and overall quality of life What We’re
Looking For - Master’s degree in Speech-Language Pathology from an
accredited program - Active state SLP license (or Educational
Limited License with supervision) - Minimum of 1 year of SLP
experience; home health or hospice experience preferred - Strong
clinical judgment, organizational skills, and attention to detail -
Compassionate, patient-centered approach to care - Reliable
transportation and valid driver’s license - Comfort working
